Empowering inclusive and independent lives

Cohome was founded to provide adults with disabilities meaningful opportunities to fully engage with their communities and lead fulfilling lives. Our person-centered programs nurture self-advocacy, independence, and meaningful connections.

Disability, Community, Friendship

Core values

Inclusion, Community and Respect represent our highest priorities and fundamental driving forces. They’re what we stand for and are at the heart of everything we do.

  • Cohome wouldn’t exist if we didn’t believe that with persistence and creativity anything is possible. Our groundbreaking inclusive housing model and transformative direct support program prove that innovation and determination go hand in hand.

  • As siblings and friends of adults with disabilities, we founded Cohome to create a new standard of excellence in both inclusion and support. Clear communication and quality service aren’t just expectations for our clients; they’re what we demand for ourselves.

  • We know that real leadership is shown through action, not words. At every level, Cohome team members are ambassadors for inclusion and hold ourselves to the highest standards of integrity and professionalism.

Ours is a story of family

Nathaniel Diskint, inspired by his younger brother Jeremy, who has Down Syndrome, sought to create a vibrant and inclusive home where Jeremy could live with peers in his community. In 2017, Nathaniel and the Diskint family founded Cohome, a 501(c)3 not-for-profit organization to advocate for inclusion through an innovative coliving model. Today, the Cohome brand has grown to include a private company creating independent living opportunities for adults with disabilities through affordable housing partnerships, as well as a direct support program to help clients achieve their independent living goals.  Our family has also grown to include many unique and wonderful residents, families, volunteers, employees, and supporters, whose efforts make Cohome the joyous community it is today.

"Never doubt that a small group of thoughtful committed citizens can change the world; indeed, it's the only thing that ever has."
